- Description
- Staff Partners Business are continuing to support a group of GP practices based in Calne. They are now looking for a Clinical Pharmacist to join them on a locum or permanent basis.
The successful candidate will be responsible for implementing an effective medicine management system within the practice, including cost-effective prescribing, working as an independent prescriber, providing advice and support for both patients and colleagues. In addition, the post-holder will also support the multi-disciplinary team, in line with the strategic objectives of the practice.
Core responsibilities (but not limited to)- Practice as an independent prescriber working within scope of practice
- Carry out face-to-face and telephone consultations with patients with minor illnesses
- To provide medication review services with patients in the practice and during domiciliary visits to the local nursing home
- To provide advice and support for patients with self-limiting conditions
- Encourage cost-effective prescribing within the practice
- Review all safety alerts ensuring the required actions are taken
- To reconcile medicines following patients discharge from hospital or clinic appointments
- Review medications for newly registered patients
- Implement and embed a robust repeat prescribing system
- To provide subject matter expertise on medication monitoring, implementing and embedding a system
- Carry out targeted medication reviews for high-risk patients
- To reauthorise medication for repeat prescribing within scope of practice
- Identify patients suitable for repeat prescribing, liaising with the relevant clinicians as required
- Review and implement safe prescribing systems for vulnerable patients, improving compliance
- Support clinicians with the management of patients suffering from drug and alcohol dependencies
- Actively signpost patients to the correct healthcare professional
- Manage the HMRA alerts and implement changes to patient medication
- Manage the Prescribing Incentive Scheme for the Practice and ensue that the Partnership is compliant and hitting agreed targets
- Manage the call and recall of patients on high-risk drugs, working with the Meds Management team to ensure that this is managed effectively
- Assist with QOF targets
- Ensure the dispensary is DSQS complaint and working towards agreed targets
- Taking a lead role in the management of mas vaccination clinics
- Administer influenza and travel vaccines
Must haves:- GPhC Registered Pharmacist
- Minimum 2 years working as a pharmacist
- Experience in managing pharmacy services in primary care
- EMIS/SystmOne/Vision user skills
This is a full-time post – Client is open to locum cover or a permanent placement
